FERPA Consent to Share Student Information with Approved Individuals
At Harts Bluff Elementary, we respect your family’s privacy while understanding that there are times when parents/legal guardians may wish to authorize other trusted individuals to communicate with the school regarding their child. Under the Family Educational Rights and Privacy Act (FERPA), we must have your written consent before we can share your child’s educational information with anyone other than a parent or legal guardian.

By completing this form, you can authorize HBE to share information about your child with specific individuals you list, for purposes such as:

• Attending parent-teacher conferences on your behalf

• Receiving academic progress reports

• Communicating about behavior or discipline matters

• Picking up school records or documentation

• Discussing special education, 504, or intervention services

• Handling routine school communication when you are unavailable

This consent ensures we can partner with those you trust, while protecting your child's privacy in all other situations.
